Our ServicesWe offer preventive, diagnostic and acute patient care in a wide range of specialties. Insurance and Payment Policies:We must have a copy of your insurance card to file any insurance for you. All insurance changes must be given to us at the time of service. If your insurance coverage changes and we are not notified, you are responsible for the charges. Your insurance company requires us to collect co-payments, deductibles and co-insurance amounts at the time of service. If you are unable to make your payment according to the terms of your insurance policy at time of check-in, you may be asked to reschedule your appointment. In the event your insurance carrier does not cover certain services, you will be responsible for any unpaid balance. Some of the insurance plans we currently participate with are:
Our staff will be glad to help you better understand what services are covered by your particular insurance policy; however, please note that due to the many health plan options available today, it is your responsibility to contact your health plan provider for information regarding benefits and or referrals.
